Reports & Statistics issues found in MetaLib Version 3.13

Description:
Several problems were found with the Reports & Statistics module in MetaLib. Fixes to these problems were made in MetaLib version 4, and to the greatest extent possible, downgraded to MetaLib v3.13 and released in Service Packs. The attached report details the problems found with each report, the changes needed to resolve each one, and when those changes were or will be released to customers.
