How do I remove a request form from the OPAC Patron Request screen?

Answer

1. In SysAdmin > Opac Configuration > Request forms > Select request form from list > Edit
2. In Select Form tab, change Opac Suppress check box if necessary
3. In Patron Groups tab, change selected or available patron groups
4. Save and exit SysAdmin

The patron request screen link is in the action box in the OPAC single record display.
