Media Equipment form shows in Tomcat WebVoyage when patron not eligible for request

Problem Symptoms:
* Patron not authorized to place Media Booking Equipment requests
* Media Equipment Booking form displays on the generic request link page
* Patron clicks form link and receives message: ?Your request failed?
Cause:
This behavior is the result of VYG-4531.
Resolution:
Issue VYG-4531 is resolved for Voyager 9.1.0 and higher.
Additional Information
Replication steps:
1. Media Scheduling System Administration > Policy Groups > Rules > Patron Group : uncheck "Patron Group Requestable using OPAC?" for patron group.
and/or
2. On Equipment tab, uncheck Equipment type Requestable using OPAC?
(Either of these steps will prevent the form from displaying in Classic).
3. Log into WebVoyage.
4. Open Patron Request page: http://{WebVoyage address}/vwebv/patronRequests
5. Note link to Media Equipment request.
