    Question

    Receive "An error occurred while adding patron information. Please consult the Circulation Desk for assistance" when submitting the Voyager Patron Self-Registration (PSR) form.

    Answer

    When using the Institution ID field in PSR, make sure new patrons are putting in an unused (unique) IID number. Using a duplicate number will cause this error to occur.
