Unknown operation" error when saving Search Engine Configuration

Problem Symptoms:
On saving Search Engine Configuration in Back Office (BE)
* Error message displayed in BE: "Unknown operation was requested - {0} "
* Error message in BE publish_server.log: Caused by: java.lang.RuntimeException: Not unique value:tableName:se_config_institution; keys:[LOW, forceBlendingParameters, threshold, Local Search Engine, LOW, forceBlendingParameters, threshold, Local Search Engine]; keyAccessors:[public java.lang.String com.exlibris.primo.domain.reference.HMappingTables.getTargetCode(), public java.lang.String com.exlibris.primo.domain.reference.HMappingTables.getSourceCode1(), public java.lang.String com.exlibris.primo.domain.reference.HMappingTables.getSourceCode2(), public java.lang.String com.exlibris.primo.domain.reference.HMappingTables.getSourceCode3()]; Existent records id:12345678,12345679
Cause:
This error may occur if there are duplicate rows in the"se_config_institution" mapping table
Resolution:
Please contact ExLibris Customer Support for assistance, referring to this KCS Article
Additional Information
The "se_config_institution" mapping table is not visible to admin users in the BE. The constraint check is done by a Java process (not Oracle) and it throws an exception when rows are duplicated.
