Hold Request not created in OPAC despite the confirmation message

Problem Symptoms: 
Hold Request submitted in OPAC receives the confirmation message, but no z37 created in the system. 

Cause: 
Mandatory Upgrade Express implementation note not performed 

Resolution: 
Implement mandatory UE change for Hold Request Confirmation Page (and New Photocopy Request Confirmation Page).
