    Description:

How to modify the search screen that appears

    Resolution:
There is very brief information in the System Librarian’s Guide – Course Reading and Reserves Version 20.0 on page 18 in the 8.5.3 Search Form for Course Reading section. It reads:

The configuration for the search form (searching for a bibliographic record in order to
add it to a Course Reading List) is defined in the ADM library:
    ./tab/pc_tab_find_course.eng

The order of the lines in pc_tab_find_course.eng determines the order they are displayed in the search screen. You can have up to eight lines in the file and notes 3 and 4 in the header show how to add comments or mark a line as non-used. Note that col 1 is the heading code from tab00.eng (and associated index code in the tab11_* files).

I think that takes care of all of your questions except "Is it possible to have the cursor default to a specific input field?". This feature is not customizable.
