    Description:
    Problem Description: form_sub_library_address is set to use an order-unit address but the claim form does not
    Actual Results: Claim form uses type 1 address instead of type 3 address as set in form_sub_library_address
    Expected Results: Claim form uses type 3 address as set in form_sub_library_address
    Reproduced on non-customer server and if not why? il-aleph02

    Resolution:
    From issue resolution: Configuration problem:

    Problem is solved by replacing the following line in ./[adm library]/form_sub_library_address :

    serial-claim-letter-01 1
