    Description:
    The staff use the Initiator Name for orders to track who requested a specific order. They want to run acq-02-01 (General Retrieval Form) to report on orders by Initiator. This works when running the report using Initiator ID, but this requires looking up the individual's ID prior to running the report. We've tried just typing a last name, first name last name, last name first name. We find that the only thing that works is the last name by itself.

    Resolution:
    Experimenting with this on our server, I find -- as you did -- that if I put in only the last name, it works.

    Though you could theoretically have multiple "Jones" or "Smith" as Initiator names, I think that these will be few enough that it will not be a problem in practice.... Please instruct staff to enter just the last name as the "Retrieve By Initiator Name" on the acq-02-01 form.
