What steps should be taken in order to run UTIL-T-2 correctly?

Resolution:
dlib dat01
stop_w
util t 2 ["Re-index database"]
start_w

However, in general, don't reindex dat01 unless told to by Support.

It locks the library for from about 30 minutes up to several hours.

While it is running, the following message will display in /M:
The Information Resource Database is being indexed. No actions are allowed until the indexing process is complete. Please try again in a few minutes.
