I'm confused about the location of the banner image for the a-z list.
Where exactly can I find the a-z_banner.gif image?

Resolution:
Requests for the AZ banner image (a-z_banner.gif) get transformed (by the script sfxadmin/gif.cgi) into requests for the file header.gif in the directory /exlibris/sfx_ver/sfx4_1/[instance]/templates/img/azlist_ver3/ where [instance] is replaced by the instance name.

If the image isn't showing up properly on your web pages and there is nothing wrong with the header.gif image itself, then there is probably an a typo in one of the Rewrite Rules for the instance in your reverse_proxy_httpd.config__ file.
