Deleted index entries remain, new entries are not added (non searchable)

Problem Symptoms:
For some record deleted index entries remain in the database and still can be found whereas for other records newly added fileds (index entries) can't be found.

Before version 22 the max number of "Words" per records for indexing was 4950. For records which exceed this limit it may happen that old index entries (meanwhile deleted) remain and still can be found or new entries are not added.
With rep_ver #019160 in Aleph 22 the number was increased to 20000 words:
Description: A word index for a bibliographic record is defined by the word number + the index type + the exact location in the record , so that a word which appears several times for the same index will be indexed several times. In several cases, the total number of words for a bibliographic record (in Z95) exceeded 5000, which is the limit of words indexes kept in memory for the record. This caused inexact words indexing when updating this record. As a solution, the number of words indexes for a bibliographic record has been increased from 5000 to 20000.
Note: the change is in internal buffers and programs only, not in the Z95 Oracle table: its data is defined as "LONG".
Implementation Notes: If the problem is relevant for a customer, he must run p_manage_01.
As you can see from the description above it's necessary to run p_manage_01 so that the old records can be changed to the new limits.

Additional Information
util f/1/28 can be used to view (and count) the words in a bibliographic record. Note that word-pairs are generated for adjacent words so the number of words is roughly twice what one might expect.
