    Library Address missing from ACQ-ORDER-CLAIM-01-08 Letter

    Problem Symptoms:
    acq-order-claim is listed in the ./xxx50/tab/form_sub_library_address, but the library address doesn't appear on the claim letter.

    Cause:
    The ./xxx50/tab/form_sub_library_address, column 2, has "3" for all the acq-nnnn forms -- but there are no address type 3 lines in the $alephe_tab/tab_sub_library_address.lng.

    The xxx50 form_sub_library_address header states:

    ! The Address Type in column 2 is matched against the
    ! address type in column 2 of the tab_sub_library_address
    ! table. If no match is found, address type "1" acts as default.

    Resolution:
    Solution 1: Change column 2 for the form_sub_library_address acq-xxxx lines from "3" to "2"

    Solution 2. Add an entry such as the following to tab_sub_library_address.lng:
