    Description:
    When you try to connect to the Web OPAC, you get the message "Library locked" even though util c/4 shows that the library is not locked.

    The www_server log has the following messages. [Note: if the www_server log does *not* have such messages, please see KB 8192-2159 which describes a more common cause of this situation.]

    SELECT - INTERRUPT BY SIGNAL.
    ....
    Oracle error: update_cursor z63.
    ORA-12571: TNS:packet writer failure.
    Oracle error: io_commit.
    ORA-01041: internal error. hostdef extension doesn't exist.
    Oracle error: update_cursor z61.
    ORA-03114: not connected to ORACLE.

    But util o/3 shows that the oracle server *is* running and you can view the vir01 z61 and z63 records in util f/4 without any problem.

    Resolution:
    We don't understand exactly what caused this, but doing util w/3/2 (Restart servers / Restart WWW servers) immediately corrected the problem.
