    Description:
    When a book is discharged, ALEPH500 seems to delete the loan record. One can see in the Item changes tab of the history pane that the item was loaned on 01/22/2005. It was returned this morning, and removed from the patron's loan list, but there is nothing in the Loans history, and (most important, in this case) the return did not reverse the lost charge. Why?

    Resolution:
    The problem is caused because Oracle was unable to write to z36h index error.
