Problem Symptoms:
Trying to request a title in Opac leads to error "Error in date from" and $LOGDIR/www_server Log shows:

Execution error : file '/exlibris/aleph/a21_5/aleph/exe/set_find_m.gnt'
error code: 114, pc=0, call=1, seg=0
114 Attempt to access item beyond bounds of memory (Signal 11)

Cause:
Upgrade Express not completed

Resolution:
On Aleph 21 server at least html file /exlibris/aleph/u21_5/alephe/www_f_lng/item-hold-request-details is still in format of Aleph 20.

In Upgrade Express 20.01 to 21.01 User Guide (Ex Libris Documentation Center > Aleph > Technical Documentation > Upgrade Express > Version 20.01 to 21.01) on page 44 ff you' find "Mandatory Changes" which must be performed.
