Description:
According to the tab_fix header help, the HOL routine is "run automatically when a HOL record is created".
This is not correct. The HOL routine in XXX60/tab/tab_fix is called only when the HOL record is created from the Items list > Tab 6. (HOL Links) > "Create New" button. It is called in addition to the INS routine.
In all the other cases of HOL record creation, only the INS routine is called.

Resolution:
Use the INS routine in Holdings library (XXX60) instead of HOL in XXX60/tab/tab_fix.
This issue is still open and was escalated to Development for further investigation <2012-01-22 01:00:01>.
