Problem Description: Variation of problem resolved by rep_change 1261.
Removed <include>include-window from the header section of file booking-page-head but when using Firefox 3.0 (or any earlier version of Firefox) double emails are generated and double entries in my e-shelf are created using the scenarios described below.

Note the problem has not been able to be replicated with IE.
Actual Results: Two emails are sent or entries created in my e-shelf
Expected Results: Only one email would be sent or entry created in my e-shelf
