find-b searches sometimes switch to ADJACENT=Y by default

From time to time, all of our "basic" searches (find-b) start using "adjacent=Y" behavior by default:

This doesn't happen all the time, and we have no idea how the default got toggled to this state. Adjacency is *not* being specified in the URLs.

How do we get the system to revert the find-b behavior back to "adjacent=N"?

Resolution:
Similar problems have been reported by other sites from time to time. As far as we have been able to determine, the system does not have a reliable default for the ADJACENT variable. At some sites they have seen that one user's selection of ADJACENT=Y has affected other users. In other places, they find that the default shifts back and forth rather arbitrarily.

Two solutions to this problem have been identified.
The first is to include the option for the user to select adjacency for each keyword search. That way, there is no default and the user can select.
The other option is to do exactly what you have done, which is to include the adjacent option as a hidden element in the html, with the value set to "N". The default USM01 screens include the option on every screen. If you choose not to do this, you should substitute the hidden variable on each screen where you have removed the option.
