We see this message in the log:.
Oracle error: io_z63_write.
ORA-00001: unique constraint (VIR01.Z63_ID) violated.
I found that the vir01 UTIL-G-2 was empty: no vir01 z52.
I ran clear_vir01 after first enabling error-logging and we see this message in the init z52 section of the clear_vir01 log:.
ORA-01045: user VIR01 lacks CREATE SESSION privilege; logon denied

Resolution:
Corrected by giving user VIR01 CREATE SESSION privilege..
