Problem Symptoms:
* For a specific bib record, when attempt to add a new item, receive "Unable to add this item" and cannot save.
* Bib has title-level holds

Cause:
Occasionally, this happens when a Library Administrative request and a patron-initiated request - both title-level - are attached to the bib record.

Resolution:
Remove one of the holds temporarily. You can then add at the item record and then recreate the hold request.
