    Description:
    There is a problem with xxx50/tab/form_sub_library_address.
    For sublibrary xxxx, the acq-m-order-slip-00 has the serials department for the return sublibrary address.
    For sublibrary yyyy, the return address is blank.
    There have been no changes to the above form or alephe/tab/tab_sub_library_address.eng for ages.

    Resolution:
    Because acq-m-order-slip is not specifically listed in xxx50/tab/form_sub_library_address, when the form prints, it defaults to alephe/tab/tab_sub_library_address.eng format 1 for each sub-library.
    In the alephe/tab/tab_sub_library_address.eng file, xxxx has a format type 1, yyyy has none.
    Either add acq-m-order-slip to xxx50/tab/ form_sub_library_address using format 3 or add a format 1 for yyyy to alephe/tab/tab_sub_library_address.eng.
    In form_sub_library_address, be sure to use acq-m-order-slip (the base form) and not acq-m-order-slip-00 (the 00 at the end is a form version number).

    Note that in order for the sublibrary address type in col 2. of ./xxx50/tab/form_sub_library_address to match a particular value in column 2 of ./xxx50/tab/tab_sub_library_address.lng (the address "Type"), the value specified in column 1 of the tab_sub_library_address.eng entry *must* be a sublibrary code.   It does *not* work in specifying a different address if column 1 is an ADM library, such as USM50.  Example:

    The following combination does *not* work in specifying that address type 3 should be used for the sublibrary MUS (and other usm50 sublibraries) acq-m-order-slip:

    form_sub_library_address:

     acq-m-order-slip        3

    tab_sub_library_address.lng:

     USM50 3 Ex Libris University Libraries
    <etc.>

    This combination *does* work in specifying that acq-m-order-slip 3 should be used for specific usm50 sublibraries, such as MUS and HIL:

    form_sub_library_address:

     acq-m-order-slip        3

    tab_sub_library_address.lng:

     MUS   3 Music University Libraries
     HIL   3 Humanities University Libraries

    <etc.>
