Resolution:
Delete the "/" from the to_blank line for the SuDocs tab_filing entry (xxx01/tab/tab_filing) (thus, permitting the "/" to be indexed). (You should leave the ":" in the to_blank, so it continues to be blanked out.)

It should look like this:
