    Problem Symptoms:
    Frequent errors in e_21 log file like:
    HANDLING DOC NO. - XXX01.002759411 08:46:11
    Oracle error: io_z00p_write - insert empty clob
    ORA-00001: unique constraint (XXX50.Z00P_ID) violated

    2010-04-06 08:46:11 An error occurred while trying to Insert doc: 002759411 into z00p table for publish set: ALL_RECS

    Cause:
    Oracle error

    Resolution:
    The z00p_id Oracle index is built on the z00p_rec_key. The error indicates that the system was trying to assign a key which already exists.

    The last_number should always be at least one higher than the highest z00p_rec_key.

    Rebuild the Z00P by doing util a/17/1 and util a/17/9/2 followed by Initial Publishing Process (publish-04).
