Subject line of email not taken from form_description.lng

Problem Symptoms:
The subject line of an emailed notice should be taken from col. 3 of xxx50 form_description.lng, but it seems that in some cases this isn't working.

Cause:
Form number omitted from column 2 of form_description.lng. (In order to have different subject lines for different form numbers, the form number must be included.)

Resolution:
Add the form number to column 2 of form_description.lng.
